The FIM is an 18-item ordinal scale, used with all diagnoses within a rehabilitation population. FIM scores range from 1 to 7 (1 = total assist and 7 = complete independence). Scores falling below 6 require another person for supervision or assistance.

What is close guarding?
A close guard is essential for when a patient is struggling with or unsure of their balance. If a therapist isn’t close by, the patient can quickly go past the “point of no return” and patient and therapist may be injured in the attempt to avoid a fall.
When do you use Alpha FIM?
The AlphaFIM® is to be completed on Day 3 after the physician orders admission to the acute care hospital setting, regardless of where the patient is physically located (e.g. Emergency Department, Intensive Care Unit (including medical ICUs after EVT and/or tPA), Stroke Unit).
